REGISTRATION FOR FAITH FORMATION 2026-2027
Thank you for taking the time to register! Before continuing below, please take note of the following to finalize the registration process:
1. Complete and Submit the Registration Form below
- In the "Comments" section, please let us know which program the child is registering for:
  - Catechesis of the Good Shepherd (PreK-6th)
  - Children's Faith Formation (Kinder - 5th Grade)
  - Sacramental Preparation (Reconciliation and First Eucharist)
  - EDGE (6-8th Grade)
  - LIFE TEEN (9-12th Grade)
  - Confirmation (10-12th Grade)
- Requirements to register for Sacrament Preparation (Reconciliation & Eucharist)
  - The child must be Baptized and at least 7 years of age and entering 2nd grade or higher
  - Completed one full year of Faith Formation or Catholic School prior to the Sacrament Preparation
  - Know what sacraments they desire to receive
  - Know basic prayers - Sign of the Cross, Our Father, Hail Mary, Glory Be
  - Regularly attend Mass on Sundays and Holy Days of Obligation
- Requirements to register for Sacrament of Confirmation
  - The student must have received sacraments of Baptism, Reconciliation and Eucharist
  - Must be entering the 10th grade or higher
  - Completed one full year of Life Teen (with good attendance) prior to the Confirmation year OR are currently enrolled at Our Lady of the Hills Catholic High School
  - Regularly attend Mass on Sundays and Holy Days of Obligation
2. After submitting this form you will be directed to the YOUTH & FAMILY MINISTRY RELEASE & PAYMENT page. You MUST complete this page with PAYMENT to finish your Registration.
- If you choose to pay by cash or check, please DO NOT use this form. Rather, you may visit the Youth & Family Ministry Office to complete registration and payment.
